An application includes all of the following:
- Completed online application form
- Personal statement (2 page max)
- Current transcript (does not need to be official and can be
emailed as a pdf attachment) from graduate students and letter
of good standing for medical students
- Two copies of a letter of recommendation
(One hard copy mailed to address below and
an e-mailed copy sent directly by the referee to
startmh@ucsd.edu)

Application and personal statements can only be submitted by
email.
Predoctoral students need to only submit graduate transcripts. First year medical students
do not need to submit transcript but must send in a letter
of good standing from the medical school's registrar's office
or other appropriate person.
No additional material (e.g. CV, resume, 2nd letter of
recommendation) will be considered. An e-mail will be sent to
confirm the receipt and process of your application materials.
